If you Out-of-Network (OON) benefits with an insurance plan not listed above, we may be able to submit the claim directly to your plan for you. Please note that you will be responsible to pay the self-pay fees at the time of visit. For more information on OON fees, please contact us.

Cancellation Policy
Appointments cancelled less than 24 hours in advance are subject to a cancellation fee of up to $100. After two (2) cancellations, the late cancellation fee will be the full visit fee.
To cancel an appointment less than 24 hours in advance, please contact our office. Cancellations within this 24-hour window are disabled on our Online Appointment Scheduler.
