If you have another insurance plan listed and have out of network benefits, we may be able to submit the claim directly to your insurance plan for you. You will be responsible to pay the self-pay fees at the time of visit. For more information on fees please contact us to discuss.
We are currently unable to accept Blue Cross Blue Shield plans.

Cancellations
Appointments cancelled less than 24 hours or 1 business day in advance are subject to a cancellation fee of up to $100. Two (2) or more late cancellations will be charged at full fee.
